APARTMENT CONDITION STATEMENT
Mandatory condition statement required to be given by the Landlord when a security deposit is taken. Allows for detailed description of the apartment condition at the start of the lease.

EMPLOYMENT VERIFICATION
Please complete the top portion of this form.  Give full employer name and address, and sign the release.  Then, give the form to your employer to be completed, and have it faxed to Symphony Properties as soon as possible.  The original form must be mailed to Symphony Properties.

GUARANTOR FORM
The landlord may request that an applicant have a qualified co-signer.  The guarantor form must be filled out completely (including social security number and salary info.), and notarized.  Please fax back the completed information portion of the form as soon as possible.  The notarized original should be mailed to Symphony Properties at your earliest convenience.

LANDLORD VERIFICATION
Please fill out ONLY the top portion, sign & date, and return to Symphony Properties.  Please be sure to include former/current landlord’s phone number.

PARKING AGREEMENT
Rental agreement for parking space, terminating upon completion of original rental period.  Please contact Symphony Properties for further information.

RENTAL APPLICATION
Standard rental application form to be filled out completely by EACH tenant applicant.  Please contact your agent at Symphony Properties for further information.

RENTAL AND SECURITY RECEIPT
Interest on the security deposit becomes due when the deposit is held more than one year. This form includes Security Deposit Calculation, detailing the deposit, interest earned and deductions for damages, unpaid rent and unpaid taxes.

STANDARD FIXED TERM LEASE
Comprehensive standard lease form, which terminates upon completion of original term.

TENANT LEAD LAW NOTIFICATION
Before renting a home built prior to 1978, the property owner and new tenant must sign two copies of this form, one for the property owner and one for the tenant.
